The Project for Assistance in Transition from Homelessness (PATH)

The PATH program provides support services for individuals who have a serious mental illness and/or substance use disorder, and are homeless, or at imminent risk of becoming homeless.

The PATH program provides support services for individuals who have a serious mental illness and/or substance use disorder, and are homeless, or at imminent risk of becoming homeless.  The PATH program connects persons 18 years or older to the services they need in order to obtain housing or remain stably housed.  The PATH program does not provide housing, but does work to connect clients to services inside Meridian and community partner resources in order to obtain affordable housing.
