Provider referrals are welcome and may be submitted in the following ways:

  • By Phone: Contact the Access Center at (352) 374-5600, or toll-free at (800) 330-5615.
  • Online: Complete the form below to submit securely to our Access Center.
  • By Fax: Send the referring organization’s forms to (352) 244-0308.

What Information Is Needed?

To process your referral efficiently, please include the following:

  • Client identification details
  • Insurance information
  • Current medications
  • Referral source details
