AOC
Adult Outpatient Counseling (AOC) provides services to adults with behavioral or emotional problems, or a longer-term mental illness that affects the ability to function effectively. Counseling services are provided in a focused, time-effective manner. In general, therapeutic services target the reduction of symptoms and enhancement or development of age-appropriate behavioral, interpersonal, or other skills needed for effective functioning in home, social, and work environments.
Bridge Program
Meridian Healthcare’s Hospital Bridge Program is designed to identify and eliminate gaps in services for individuals who present to the hospital with emotional well-being and/or substance use challenges. Our peer specialists collaborate with community partners to better meet the needs of clients, whether it’s at their bedside or within the community at large. A needs assessment is conducted to provide a holistic approach to care. Following an assessment, the peer specialist shares various resources to aid patients on their recovery journey. Support continues until satisfaction is met or the necessary programs are located.
CAT
The Community Action Team (CAT) program is designed to enable youth facing challenges in Baker, Bradford, Columbia, Dixie, Gilchrist, Hamilton, Lafayette, Levy, Nassau, Suwannee, and Union counties the opportunity to receive the support needed to succeed in their own community. The program goal is to help youth reach their fullest potential while remaining in their home. Each client’s family is provided with a team that includes a therapist, psychiatrist, nurse, care coordinator, and a mentor to provide support through all facets of treatment. During the treatment process, the client and family receive therapy, while the client receives additional, individualized support from a care coordinator and mentor.
CCBHC
The CCBHCs are designed to provide vulnerable individuals with access to comprehensive behavioral healthcare, including crisis services, regardless of their ability to pay. CCBHCs integrate physical health with a range of mental health and substance use treatment and support services.
Meridian’s clients will experience greater integrated care by providers sharing treatment plans and engaging in coordinated decision-making that is rooted in compassionate person and family-centered care.
CHOP
Meridian’s Children’s Outpatient Children (CHOP) provides services to children, adolescents, and families. Services delivered include: individual, group, and family therapy in our clinic, and in the community, such as a school.
Masters-level clinicians use proven techniques to evaluate and treat a wide range of behavioral problems, such as depression, anxiety, adjustment, trauma, and substance use disorders.
CWCC
The primary goal of Child Welfare Care Coordination is to ensure seamless management of services to meet the needs of children and families, thereby ensuring access to resources and support to enhance positive outcomes.
A care coordinator assists in enhancing and expanding services by providing wraparound support to individuals who are referred to Meridian Healthcare from child welfare agencies. The care coordinator coordinates admissions, discharges, and follow-ups with children as well as families; thereby, offering engagement and support in a seamless manner.
Diversion & Recovery
The Diversion and Recovery team at Meridian focuses on diversion potential for cases with extensive incarcerations or being committed to forensic state hospitals. Once defendants have been identified with mental health and substance use challenges or treatment needs, Forensic Specialists assist them and their attorneys with criminal proceedings. Ongoing competency restoration training and education are provided in the jail and the community for each defendant. Forensic Specialists monitor misdemeanor and felony charged clients. The program focuses on supporting individuals so they can remain in the community.
FACT
We are Florida Assertive Community Treatment (FACT), a team of community-based professionals who provide individualized, ongoing care to those who struggle with mental health challenges. Our multidisciplinary team of compassionate experts includes psychiatric nurse practitioners, nurses, behavioral health clinicians, and care coordinators.
FMT
The Forensic Multidisciplinary Team (FMT) provides wrap-around services to clients with pending felony charges. Services provided include care coordination, individual therapy, medication management, peer support, and competency restoration training. FMT also provides on-call availability for emergencies.
FTC
The FTC program is to assist those within the following counties: Alachua, Baker, Bradford, Union, Columbia, Suwannee, Hamilton, Levy, Dixie, and Gilchrist the opportunity to succeed in their own community. The program goal is to link clients involved with DCF/PSF to substance use services, as well as other community services that are needed to aid in the reunification of their family.

Meridian Healthcare is here to meet the needs of those affected by mental illnesses and substance use disorders throughout North Central Florida. Our broad services are designed to meet you where you are and offer evidence-based, individualized, and appropriate care tailored to your unique needs. Services are available remotely throughout the state of Florida via Telehealth, and in person at one of our 13 locations.
MAT
Medication-Assisted Treatment (MAT) uses an evidence-based practice that combines pharmacological interventions (medication) with substance use disorder counseling in individual and group sessions for those struggling with addiction to opiates and/or alcohol. MAT is an outpatient service and is available
throughout Meridian’s service area.
MHFA
Mental Health First Aid (MHFA) is the initial help given to someone experiencing a mental health or emotional crisis before professional help is obtained. In the MHFA course, you learn risk factors and warning signs for mental health and addiction concerns, strategies for how to help someone in both crisis and non-crisis situations, and where to turn for help.
MIST
Mothers’ Intensive Supportive Treatment is a residential treatment program designed for pregnant women and mothers struggling with addiction and co-occurring mental health concerns. They offer a safe place for expecting and new moms with babies to access appropriate care for themselves while continuing to establish strong bonds with their children, including those trying to regain custody.
MRT
The Mobile Response Team program is designed around youth facing emotional well-being challenges and young adults (25 and younger), within Baker, Bradford, Columbia, Dixie, Gilchrist, Hamilton, Lafayette, Levy, Suwannee, and Union counties, who are in need of 24/7 crisis intervention services.
The program’s goals are to lessen the trauma of crisis situations, particularly diverting individuals away from ERs and/or jail, and stabilizing them in the most conducive setting possible.

Primary Care
For those who suffer from a substance use and/or mental health disorder in addition to any medical issues, Meridian’s primary healthcare program is right for you. Research shows that when physical and mental health are treated together, you can experience improved outcomes. Meridian Healthcare integrates medical and psychological treatment by providing a comprehensive primary healthcare clinic.
SAPP
Substance Abuse PICC Program (SAPP) is a hospital diversion program for patients who have a history of IV drug use and a severe infection that requires antibiotics through a PICC line. Instead of the patient sitting in a hospital bed for 4 to 6 weeks, they will reside at Meridian’s Residential Substance Abuse Program, where they will receive 24/7 care and start on the road to recovery. This residential setting provides a structured, drug-free environment in which individuals learn about the disease of
addiction, engage in supportive counseling, and receive on-site home healthcare services for their infusion
supplies, and will be educated on maintaining a clean working PICC line.
SSVF
The goal of the Supportive Services for Veteran Families (SSVF) Program is to help Veteran families who are homeless or at risk of homelessness to quickly gain stability in permanent housing.
