As a Primary Care patient, you have an alternative to emergency room treatment for their chronic conditions with providers who know you by name and understand your unique needs. Your primary care doctor can help with physical needs such as hypertension, diabetes, high cholesterol, and various other physical issues, and Care Coordinator are available to help you understand and manage your diagnoses in order to live a happier, healthier lifestyle.
