Family Member Rights

With permission from your loved one (18 years of age and older), family members can be present during the intake process.

With permission from your loved one (18 years of age and older), family members can be present during the intake process. Family members wishing to ensure access to health information and status updates while their loved one is in treatment will need an Authorization of Release of Information signed by the patient granting access. Without signed permission, Meridian staff may not, by law, share any information with you. However, family members may provide vital information, such as onset of symptoms, current and past medications, etc., without signed consent. Other access documents include Power of Attorney for Healthcare, Declaration for Mental Health Treatment and Guardianship documents. When possible, family members may want to consider securing these documents in advance of the need for evaluation and treatment in order to facilitate the process and provide access to information.
