Care Coordination

Care Coordination is available to adults and children with serious mental illness whose needs include coordination of care relating to health and social services. Our telehealth capability ensures access to experts throughout our Meridian’s service area. Clients in need of intensive services to address combined mental health, substance abuse and trauma are served by our Day treatment and intensive outpatient services.
